Why was my carrier deactivated?

Most likely one of: (1) missed MCS-150 biennial update, (2) lapsed BMC-91 insurance (insurer canceled or non-renewed), (3) voluntary deactivation by you or a prior owner, (4) FMCSA enforcement action following audit failure or compliance review, or (5) extended inactivity. The carrier-status detail in SAFER usually shows the specific deactivation reason code.

SAFER's carrier-status field surfaces the specific deactivation reason for most cases. Look for the "Operating Status" field — codes like "INACTIVE - Mandatory Insurance Filing Required" mean lapsed insurance, "INACTIVE - MCS-150 Update Required" means missed biennial, "INACTIVE - Out of Service" means an OOS order is on file, etc.

For deactivations without a clear SAFER reason code, the next step is to call FMCSA at 800-832-5660 with the USDOT and ask the agent to look up the carrier-status history. The agent can typically provide the specific deactivation date, reason, and any required corrective filings.

Some deactivations are inherited — a prior owner of the entity voluntarily deactivated, or a name-change/ownership-change event broke the FMCSA record continuity. These cases sometimes need a clarification filing with FMCSA before reinstatement can proceed.

For most owner-operators and small fleets, the cause turns out to be missed MCS-150 (the most common) or lapsed insurance (second most common). Both have straightforward fixes through our standard reinstatement filing.
